✅ Description:
Mirogabalin, a derivative of gamma-amino acids, selectively binds to the α2δ subunit of voltage-gated calcium channels (VGCCs). Reducing calcium influx into presynaptic neurons suppresses neurotransmitter release and decreases CNS hyperexcitability.
- Absorption: Rapid, peak plasma levels within 0.5–1.5 hrs
- Protein Binding: ~25% (low)
- Distribution: 64–88 L
- Metabolism: Mostly unchanged via renal excretion; 13–20% by UGT enzymes
- Elimination Half-life: 2–4.9 hrs
- Excretion: ~99% through urine

Mirogabalin Besylate
✔️ Dosage & Administration
Adults: Start with 5 mg twice daily, titrate weekly by 5 mg to a maximum of 15 mg twice daily.
Renal Impairment:
Mild: 5 mg BID → up to 10 mg BID
Moderate: 2.5 mg BID → up to 7.5 mg BID
Severe: 2.5 mg once daily → up to 7.5 mg once daily
Children/Adolescents: Safety not established
Hepatic Impairment: Mild–moderate safe; no data for severe

↑ Plasma levels with Cimetidine, Probenecid
CNS depression potentiated with Lorazepam or alcohol
Substrate of OAT1, OAT3, OCT2, MATE1, MATE2-K, UGT → inhibitors may increase exposure
Food: No significant impact

Patients with significant hepatic or renal impairment

Excreted in breast milk – weigh maternal benefit vs infant risk.
