Mirogabalin, a derivative of gamma-amino acids, selectively binds to the α2δ subunit of voltage-gated calcium channels (VGCCs). Reducing calcium influx into presynaptic neurons suppresses neurotransmitter release and decreases CNS hyperexcitability.
Mirogabalin Besylate
Adults: Start with 5 mg twice daily, titrate weekly by 5 mg to a maximum of 15 mg twice daily.
Renal Impairment:
Mild: 5 mg BID → up to 10 mg BID
Moderate: 2.5 mg BID → up to 7.5 mg BID
Severe: 2.5 mg once daily → up to 7.5 mg once daily
Children/Adolescents: Safety not established
Hepatic Impairment: Mild–moderate safe; no data for severe
↑ Plasma levels with Cimetidine, Probenecid
CNS depression potentiated with Lorazepam or alcohol
Substrate of OAT1, OAT3, OCT2, MATE1, MATE2-K, UGT → inhibitors may increase exposure
Food: No significant impact
Hypersensitivity to Mirogabalin
Patients with significant hepatic or renal impairment
Common: Somnolence, dizziness, edema, weight gain
Serious (rare):
Loss of consciousness, confusion
Liver dysfunction (jaundice, nausea, vomiting)
Visual disturbances (blurred/double vision)
Use only if benefits outweigh risks (animal studies show placental transfer).
Excreted in breast milk – weigh maternal benefit vs infant risk.
Store below 30°C, in a dry place away from light & moisture.
Keep out of reach of children.
